Do you qualify for medical cannabis?

Patients may qualify for medical marijuana in the state of Florida if they are a current Florida resident and have been diagnosed with the following conditions:

  • Cancer
  • Terminal condition
  • Epilepsy
  • Glaucoma
  • ALS
  • Crohn's Disease
  • Parkinson's Disease
  • Multiple sclerosis
  • HIV / AIDS
  • PTSD
  • A terminal condition diagnosed by a physician other than the qualified physician issuing the physician certification.
  • Chronic nonmalignant pain.
  • Other debilitating medical conditions of the same kind or class as or comparable to those enumerated, and for which a physician believes that the medical use of marijuana would likely outweigh the potential health risks for a patient.

Procedure Overview

Step 1:
Submit Medical Records

Request or gather your medical records, reports, prescriptions that are proof of your diagnosis consistent with the need for medical marijuana. You may download our Medical Release Form to request medical records from your doctor.

Once our office receives your medical records, we will contact you to schedule your initial visit with Dr. Wong.

Step 2:
Visit Our Office

On your initial appointment, Dr. Wong will discuss your medical history, current medications, and perform a physical examination. You can download and complete our New Patient Registration Form ahead of time. 

If you are qualified, your information will be registered into the Office of Medical Marijuana Use Registry.

Step 3:
Apply for Registry Card

Using your patient registry number, you will need to complete the Registry Card Application online. Patients will need to upload a photo, proof of Florida residency (Driver’s License or Utility Bill), and pay $77.75 online with credit card or bank account information.

Step 4:
Receive Your Recommendation 

After 2 to 3 weeks, the patient will receive their temporary approval via email and can purchase their medication while waiting on the Medical Marijuana ID card to be mailed.
