Patient Qualifications

In Florida, patient must have one of the following conditions to qualify for their medical marijuana card.

  • Cancer
  • Terminal condition
  • Epilepsy
  • Glaucoma
  • Amyotrophic Lateral Sclerosis (ALS)
  • Crohn’s Disease
  • Parkinson’s Disease
  • Multiple Sclerosis (MS)
  • HIV / AIDS
  • Post Traumatic Stress Disorder (PTSD)
  • A terminal condition that is diagnosed by a physician other than the qualified physician issuing the certification.
  • Chronic nonmalignant pain.
  • Other debilitating medical conditions of the same kind or class as, or comparable to, those enumerated and for which a physician believes that the medical use of marijuana would likely outweigh the potential health risks for a patient.

Visit Our Office

On your initial appointment, Dr. Wong will discuss your medical history, current medications, and perform a physical examination. If you are qualified, your information will be registered into the Compassionate Use Registry.

Apply for the Registry Card

Patients will verify their Florida ID or Driver’s License and pay $77.75, online, with credit card or bank account information. If the patient is a seasonal resident, other forms of proof of residence will be accepted.

Initial Consultation

During this in-person visit we will discuss your medical history and present symptoms. Your personal information and dosage recommendations will be placed into the Florida Registry. Our doctor will discuss your treatment plan with you.​

Recertification Visit

$150
As required by the state, the doctor must see the patient in person or via telehealth to reevaluate every 210 days, the maximum time of physician’s orders. In this visit, Dr. Wong will discuss the current regimen and symptom response. Then, the order recommendations will be placed for the next certification period.

Pediatric Patients

The initial visit is a one-hour consultation with Dr. Wong to review the patient’s medical history and discuss treatment options. Includes on-going guidance to make treatment adjustments to find the best regimen for each individual patient.
